Convenience Services Membership
These forms are made accessible for both SECU members and potential members use! Since signatures are required on these applications, the on-line access allows you to download the application, complete it, and send it in. The membership application requires a clear and legible copy of either your drivers license or passport and social security number or proof of application. Please read all associated disclosure statements.

- Change Account Information Change Address, Add or Change Services (Call24, Checking, Net24, Plasticheck, Savings (Christmas Club, 55Plus, Secondary), etc.)
- Checking Adding a Checking Account.
- Checks Use for 1st check order or when opening a new checking account, or reordering with changes to your account information
- Checks: Reordering or call 877-585-8777, ext. 0 if you have a reorder ID# and your address or ownership has NOT changed, otherwise contact SECU
- Consolidation - Fast Track Loan Application
- Direct Deposit Depositing full paycheck into your account

- Identity Theft In the event of Identity Theft, completed form required to have accounts closed and reassigned.

- New Director Application If you are interested in serving as a Director on SECU's board, complete and mail/FAX form to SECU.
- Overdraft Protection Use to originate or update your Overdraft status
- Payroll Deduction Depositing part of paycheck into your account
- Privacy Opt Out Use this form to opt out of Internet, Telephone, or Internet Access to your account(s).
- Stop Payment For stopping payment on checks (Personal, Cashier, Teller/Bank) or ACH payments
- VISA Automatic Payment Form Use this form to setup automatic Visa payments from your account.
- VISA First Credit Card Application Use this form for Platinum, Gold, or Classic cards, if you are a first time card holder, and have a Social Security Number and U.S. address.
- VISA First Credit Card Application Use this form for Platinum, Gold, or Classic cards, if you are a first time card holder, and do not have a Social Security Number
- VISA Credit Joint Cardholder or Authorized User Addition Use this form if you currently have a VISA credit card and would like to add a joint card holder or Authorized User.
- VISA Credit Card Change Request Use this form to request Name Change, Limit Increase, or Upgrade to Gold or Platinum, if you are not a first time card holder.
